Accepted Insurance Plans

Insurance plans vary and may cover 0%-100% of your medical costs. You are responsible for any amount not covered by your insurance plan. We are happy to submit claims to your insurance company, but you should direct any questions or complaints regarding your coverage to your insurance carrier. We are not responsible for any rejections by your insurance plan. Any dispute over the amount your plan paid is between you and your insurance company and you are responsible to pay your portion to us while you work out your dispute with your insurance.

All co-payments, deductibles and other amounts not covered by insurance are your responsibility. You should be prepared to pay these amounts at the time of your appointment. If you do not know what your co-pay is or how the insurance will cover your medical care, please call your insurance carrier prior to your appointment so you will have this information when you arrive at the office for your visit.
